  USA TODAY Network's Jason Pohl embedded on the Minden mission to give readers a first-hand account of what this humanitarian crisis looks like with stories, compelling videos and striking photos of the rescues. Since January, more than 4,600 people have been killed crossing the Mediterranean to Europe, making 2016 the deadliest year on record, according to the United Nations refugee agency. The death toll could be higher because of bodies never found or deaths never reported.
